MEMBERSHIP

• Your initial membership rate is guaranteed with continued enrollment.  A restarted membership will pay the current rate.

• A one month written notice is required to cancel month-to-month memberships. One additional month will be charged.

• Leave of Absence/Hold requests must be in writing.  One additional month will be charged and held for use upon the stated return date.  Term memberships will be frozen and then automatically reactivated upon the stated return date.

• Your membership is non-transferable.

• Credit or debit card payment methods used to enroll or pay ongoing membership dues are subject to a 3% surcharge.

• No refunds on membership will be provided.

SAFETY

Understand that there is a risk of personal injury in the course of participation and with this knowledge, you agree to assume the risk of any injury and damage during and directly after training. 

Specifically, you agree to hold harmless Deseo Studios, and all other individuals, organizations, sponsors, promoters, operators, hosts, instructors, associations, schools, owners, officials, directors, employees and other participants connected with the event from all losses, damages, injuries, causes of actions, claims, or complaints in the event that the you are injured in any way during the participation, instruction and/or performance of any exercise or during any activity associated with the event location, off site activity venue, or during transit to or from the event.

You understand that you must be in good physical condition to participate in studio activities.  You further agree to strictly obey instructors communications, observe safety rules, and dress in appropriate attire for the activity.

MEDIA RELEASE

You agree that any pictures, audio, or visual recordings taken of you in connection with Deseo Studios can be used for publication, promotion, articles, shows and advertisement without additional consent and without compensation at this time or any other time.

ACKNOWLEDGEMENT

You have read and understand this release and agreement and agree to its provisions. You are not under the influence of any drugs, alcohol, or other intoxicants. You are not suffering from any illness or incapacity. You are over 18 years of age. (If not over 18 years of age, parent or guardian must sign.)
